Grantee Description

The Santa Clara Valley Health & Hospital System Mental Health Department provides support and oversight to the KidConnections, a group of not-for-profit and public agencies who provide high quality, trans-disciplinary screening, triage, assessment and consultation services for young children with suspected and/or known delays, diabilities and/or behavioral concerns. As part of the FIRST 5 System of Care, the Department also provides/contracts for mental health services for children up to 5 years old and their families in the targeted Power of Preschool sites and community partnerships, and/or to families and young children who are Medi-Cal eligible.
