FIRST 5 Santa Clara County’s System of Care is a network of coordinated and integrated partnerships, services and activities aimed at improving the lives of children prenatal through age 5. FIRST 5 funds are used to enhance and fill in the gaps in services so that there is a seamless system for families and children in the County. The primary goal of this System of Care is to ensure that all levels of the community are engaged in and educated about the importance of the first five years, ultimately making young children their highest priority.

Court Services FIRST 5 and its partner agencies provide services and supports for families with children through age 5 involved in the courts system. More		Differential Response FIRST 5 and community partners collaborated to develop a 4-tiered Differential Response System to respond to child abuse and child welfare system referrals through the Department of Family and Children’s Services (DFCS). High Risk Infant Follow-Up Program FIRST 5 funds a specialty program that follows up with infants discharged from the Neonatal Intensive Care Unit (NICU) at the Valley Medical Center. Early Screening and Assessment FIRST 5 is providing a coordinated system of screening, referral, triage, targeted assessments and development of appropriate care plans for children with developmental delays and serious behavior concerns. More		Community Engagement and Education FIRST 5 and its partners are committed to raising community awareness of the importance of the healthy development of all children through age 5 through education and communications campaigns. Community of Learning FIRST 5 provides program specific cross-disciplinary learning opportunities to representatives from each partner agency, and across all community partnerships. More		Family Support Services FIRST 5 funds a variety of programs to ensure that all families and caregivers get the support they need to face challenges of raising healthy young children. More
